  Description of entity and state/country of incorporation
• If applicant is a natural person:
  The applicant's I/C or Passport no. and nationality
• Inventor's full name and address
• 1 copy of the Specification in English (a description, a claim or claims, drawings, abstract)
• Details of Priority Claim

• Appointment of Agent Form signed by the applicant
• Statement Justifying Applicant's Right to the Patent (where applicable)

• Applicant's full name and address
• If applicant is a legal entity:
  Description of entity and state/country of incorporation
• If applicant is a sole proprietorship / partnership:
  Full name, ID no. and nationality of sole proprietor/of each partner
• If applicant is an individual:
  Full name, ID no. and nationality of individual
• 7 clear representations of the mark
• Classification and Specification of goods and/or services
• Priority Date and Data (where applicable)
• Limitations / Conditions (where applicable)
• First date of use in Malaysia (where applicable)

• Notarised Statutory Declaration affirmed by applicant as bona fide proprietor of mark
• Certified Priority Documents in English (where applicable)
• Certified English translation and / or transliteration (where applicable)

  For repeating surface pattern: each representation showing a complete pattern and sufficient portion of the repeat in length and width of size not less than 18cm x 13cm

• Name of Article / Set of Articles, and International Classification
• Statement of Novelty
• Author's full name and address (where applicable)
